I first tried to log in on my Bellin app but always encounter a problem. I did use MyChart to check in. The Bellin app should redirect one to MyChart

I have enjoyed the relationship with Dr. Fuglestad for the last I don't know how many years- but it has been a long time. I will really miss his warm and compassionate care that he had. I wish him well in his retirement!

My initial appointment was cancelled the day of (2 hours before appointment time). Not the best thing for the patient but I do understand it does happen.

Experience was good. Emplify overall is not. Worse online experience, billing, service, everything.

When I was told I would have labs, they should be specific - is it fasting blood work or not. Will I be doing a urinalysis. This would help me prepare

I was never told about my chart or an app. I would like a follow up on my availability on that information.

I really enjoy working with Dr. Fuglestad and the only problem, if you will, that I have is sometimes it's very difficult to get in to see him, so I wind up going to urgent care. And that has been a good experience, but I would feel much better if I was sitting down with Dr. Fuglestad. Thank you very much.

In general, excellent. I thought I had everything completed prior to my last visit, but I still had man questions to be answered after I arrived.

I have been coming to this office for more than 20 years now. Wouldn't change a thing.

Doctor, explained my options to me and why he thinks this was the best way to attack it at that. He asked if I was putting external pain reliever but I said I can't reach it. He suggested using a patch. Great idea.

I have many appointments and having to answer the same questions over and over again is very, very, very tedious.

First time seeing Dr. Fuglestad. He was knowledgeable, courteous and did his examination in a calm, clear and pleasant manner. I was there for a relatively simple check up - but he didn't rush and was very thorough.
