You folks keep doing what you're doing. I am my family couldn't be happier with the care that I have been receiving over the last two weeks. I have all the confidence in the world in my Mellen health providers. Thank you for being you.

I am unable to use MY CHART

My provider made several statements about my record/medical history that were inaccurate. When I corrected her she acknowledged her mistakes. This visit accomplished nothing and was a waste of my time.

Answered all question with expertise. also asked good personal questions.

I was in there last week for my annual checkup. I left specific instructions for my blood test results of how I wanted to receive them. They were not carried out anywhere close to what I asked for. This is unbelievably unforgivable. I would like the person who wrote a note in my file, who really had a problem with my instructions, which I believe was Anne, that that person be reprimandative, not fired. I still to this day have not received my lab results. As far as I'm concerned, you should not be charging me for any of this.

It was fabulous. This was my first experience in the hospital. And for everything that happened, they're perfect. You know, I even love life even more. You know, people do care. And Berlin Health, yes. All the way. Thank you.

You need to take a long hard look at your Epic check in system. Way too many repetitive questions. Not a user friendly system! The entire staff at all levels were outstanding!

The trainee did a great job. And Kristin-does a great job as always.
